Full Membership is open to licensed physicians (MD, DO, or DPM) who desire to use the services of The Cooperative and who agree to abide by the bylaws and policies of the cooperative and accept the responsibilities of membership.

Full Membership will be granted upon submitting all required credentialing information to certify that the person is a physician fully licensed and able to practice medicine within the US to the digital identity certification platform, certification of these credentialing documents by a third party, and having paid the required membership fee.

By purchasing this option, you acknowledge that you are a fully licensed physician, able to practice medicine within the United States with an unrestricted license in any state, and you are not subject to any sanctions or legal actions that would impair your ability to practice.

There are no refunds for memberships if you are found to be disqualified for membership.
